Pre-Sale Boutique Tickets

  • Boutique items valued under $50 are $1 boutique tickets.
  • Boutique items valued $50 and over are $5 boutique tickets.
  • Pay by Check, Mastercard or Visa.
  • Payment must be received before Friday, October 31, at 10:00 am.
  • You do not need to be present to win.
INSTRUCTIONS:
  1. Check the box next to the boutique item(s).
  2. Type in the number of tickets you'd like to purchase for that item (Boutique Tickets are $1.00 and $5.00).
  3. Add up your total for each boutique item and enter it in the "Total Amount" box.
  4. Please add up the total amount due for all boutique items and enter it in the "Amount Due" box.
  5. Once you've chosen your boutique items, click the "Submit" button. This will take you to the Pre-Sale Boutique Tickets Mail-in Form.
